Summer 2023 Registration

All Children must have these forms completed to attend our summer program, regardless of prior attendance.

Summer Registration for Elementary Program (Grades K-4) (Elementary Aged Attendees must be sure to complete both the summer registration form and the VBS registration. If your child is in Grades K-2 you will also need to complete the Honey Bee consent form below.)
Summer Registration for Warehouse Program (Grades 5-7) 
VBS registration for the Elementary Program (Grades K-4)

Medical Release (All Grades)
Consent to Participate in Honey Bee Keeping Activities (All Grades)
Authorization for Disclosure of Confidential Information (All Grades)

Your child will not be accepted until ALL the forms for their grade level are completed.
